Directions to Teachers:
1. If a student wants to enter on multiple voice parts, enter his/her information twice. Fee is $10 per entry.
2. Mail a hard copy of this form, a copy of your membership card, and your school’s check made out to District IX Vocal Account to Paula Vickers, Vocal Chair.
3. Director’s signature indicates that he/she has a signed Parental Consent Form for Release of Student Information from all students auditioning. These forms must be turned in to the event chair. In addition, the director is verifying that all students have parental/guardian permission to audition and participate in this event.
